Update Personal Information
   
Q1. I need to notify Verizon of my name change. Who do I contact?
   
A1. You may contact the Verizon Benefits Center at 1-877-4VzBens
(1-877-489-2367). At the main menu select “Healthcare”, then select “Review or Change Coverage”. Representatives are available from 8:00am to 6:00pm EST, Monday – Friday.
   
Q2. I need to update my address information. Who do I contact?
   
A2. You may update your address information online. Just log on to Your Benefits Resources at www.verizon.com/benefits (user ID and password required), and click on “Personal Info”. You may also contact the Verizon Benefits Center at 1-877-4VzBens
(1-877-489-2367). At the main menu select “Healthcare”, then select “Review or Change Coverage”. Representatives are available to assist you from 8:00am to 6:00pm EST, Monday – Friday.
   
Q3. I need to update my beneficiary information. Who do I contact?
   
A3. You may update your beneficiary information online. Just log on to Your Benefits Resources at www.verizon.com/benefits (user ID and password required) and click on “Beneficiaries”. You may also contact the Verizon Benefits Center at 1-877-4VzBens (1-877-489-2367). For Life Insurance beneficiaries, at the main menu select “Healthcare”, then select “Review or Change Coverage”. For Pension beneficiaries, at the main menu select “Retirement and Pension”, then select “Pension”. Representatives are available to assist you from 8:00am to 6:00pm EST, Monday – Friday. If you are looking to update your Savings Plan beneficiary, at the main menu select “Savings Plan”.

Q4. How can I obtain verification of my previous employment with Verizon?
   
A4. You may obtain verification of your previous employment by calling The Work Number at 1-800-996-7566 or visit their website at www.theworknumber.com. You will need your social security number and the Verizon company code (10303). If you have been separated from the company for more than 5 years, you must mail or fax in your request. You may fax your request to the Employee Service Center at 972-718-7388. Mail may be sent to: Employment Verification,
700 Hidden Ridge, Irving, Texas 75038.
   
Q5. I have employment verification forms that require a signature from Verizon. Where can I send the forms?
