As of January 31, 2005, Verizon mailed 2004 income tax statements to retirees’ homes. Depending on your situation you could receive more than one tax statement. Some of these situations include, but are not limited to, changes to your distribution code during the year or moving to a new state during the year.

The information in the letter to your home contains a table called, “Which Tax Statement(s) Will You Receive?” to help you in determining the statements applicable to you. In addition, samples of Form 1099-R and Form W-2, along with an explanation of what is reported on each form are included with the letter.
